The reasons may be twofold-1. no time 2. anatomical "operables" have rather obvious "insert/penetration" zones that are better disguised by clothing.

I think "no time" wouldn't be a major concern as I'd imagine any medicus would simply rip the pants up the leg. It would take about two seconds, they must've gotten good at it
